| Abstract | 「Abstract」円板状半月板は本邦を含むアジア圏で頻度の高い疾患であり, 圧倒的に外側に多い. その発生機序や長期予後についてはまだまだ不明な点が多いが, 活動性の高い若年者での円板状半月板損傷が多く, 有症状の円板状半月板損傷は手術適応となる. 一方, MRIで無症候性の円板状半月板が発見されることも多く, これらは通常は経過観察となる. 従来, 円板状半月板はその形状, 付着部形態から完全円板状半月板, 不完全円板状半月板, Wrisberg型として分類されていたが, 近年Ahnらが提唱したMRIによる4つのタイプ分類が臨床的にも実用的である. 手術は形成的切除術に加え温存した辺縁部に断裂が残存すれば追加縫合を行い半月板の温存を試みるほか, 変性や逸脱の見られる例ではセントラリゼーション法を追加し逸脱の予防・整復を行う. |
