| Abstract | 「Abstract」肩甲骨骨折に関する最新の診断, 治療について総論, 部位別に分け, 詳述した. 総論ではまず解剖, 頻度, 分類, 受傷機転, 診断について述べた. 次に, 治療に難渋する肩甲帯部重複部損傷において, 治療法の選択の指針となるGossのSSSC理論と仲川の2階建構造理論を紹介した. 各論では, 関節窩骨折で主に現在行っている鏡視下, 鏡視補助下の手術(type Iは鏡視下に, II, IIIは主に鏡視補助下に, IV, V, VIは直視下手術の補助に)の実際について述べた. 頚部, 体部, 肩峰, 肩甲棘骨折については, 保存療法が主体であること, 手術適応, 手術の実際について述べた. 最後に烏口突起骨折は分類, 手術適応, 小切開による基部骨折に対する手術手技について述べた. 肩甲骨骨折は理解しにくい骨折の一つであるが, 骨癒合が良好で非荷重関節であるため, 病態を理解し治療すれば良好な治療成績が得られる骨折である. |
