| Abstract | 「Abstract」第5中足骨疲労骨折(Jones骨折)はアスリートの下肢に好発するスポーツ損傷の一つである. この骨折を予防するためには, 日本における疫学やリスクファクターを知り, 予防戦略を立てる必要がある. また, この骨折は保存加療に抵抗性で再骨折も多いことから, 第5中足骨骨折を診たときには, それが急性外傷によるものなのか, 疲労骨折であるかの鑑別のもとに治療方針を立てる必要がある. 疲労骨折の場合は基本的には手術加療を選択することが多いが, この骨折は手術合併症が発生しやすいことでも知られており, 特に偏心性のスクリュー刺入は, リーミング時のthermal necrosisの誘因となるため慎重な手術操作が必要である. また, 術後にも再発例が認められるため, リハビリテーション期間中に選手ごとに可変性のリスクファクターを見つけ, 介入することが非常に重要である. |
