| Abstract | 「Abstract」環軸椎回旋位固定(atlantoaxial rotatory fixation; AARF)は, 就学前後の小児を中心に多くみられる斜頚, 頚部痛, 頚椎可動域制限の三徴を示す疾患である. 原因は軽微もしくは強い外力によるもの, 頚部周囲の炎症性疾患や手術後であることが多い. 多くは保存治療に反応し予後良好であるが, 遷延化すると治療期間, 侵襲の面で患者負担は増える. 発症後1週以内であれば, 臥床安静, カラー固定, 投薬治療で経過観察し, 発症後1週を過ぎた時点で改善がみられない場合は持続牽引治療を開始する. 発症後1か月を過ぎた症例でも持続牽引治療は有効で, 麻酔下徒手整復+ハローベスト固定や脊椎固定術は牽引治療で整復不可能, 整復後再発を繰り返す症例が対象となる. 早期診断, 早期治療開始が最も重要である. |
