| Abstract | [Abstract] 肩後方アプローチに必要な解剖学的構造を解説する. 体表から触知できる肩甲棘には, 僧帽筋の中部・下部線維と三角筋の後部が付着しており, これらを骨膜下で剥離すると, それぞれ棘上筋と棘下筋が明らかとなる. 肩甲骨頚部・後方関節内へのアプローチには, さらに棘下筋と小円筋の筋間を鈍的に分離し, 後方関節包から関節内を確認できる. 肩甲上神経が棘窩切痕で障害されている場合, 切痕前方の確認には棘上筋を前方へと牽引し, 切痕後方の確認には三角筋を肩甲棘より剥離した後, 棘下筋を下方へ翻転して肩甲上神経と血管束にアプローチする. |
