| Abstract | [Abstract] 肩腱板損傷に対する手術治療は関節鏡視下腱板修復術が主流となってきているが, 直視下アプローチは現在でも広範囲断裂や一次修復不能断裂に対する直視下腱板修復術および筋腱移行術, 上腕骨大結節骨折や小結節骨折に対する骨接合術などにおいて使用されており, 習得すべき手術進入路である. 棘上筋腱から棘下筋腱に進展する後上方型の腱板損傷においては三角筋間アプローチが用いられ, 広背筋移行を追加する場合は腋窩部のアプローチを追加する. 棘上筋および肩甲下筋の前上方型腱板損傷においては大胸筋・三角筋間アプローチが用いられ, 修復不能な腱板断裂では大胸筋移行の追加を検討する. 肩甲下筋腱から棘下筋腱に至る三腱断裂では同一皮切から両アプローチが併用されることもある. 本稿では肩腱板損傷に対する直視下腱板修復術および筋腱移行術に必要なアプローチに関して, 後上方型の腱板損傷と前上方型の腱板損傷に分けて解説する. |
