| Abstract | 「Abstract」胸郭出口症候群 (thoracic outlet syndrome : TOS) に対する手術は, 経腋窩アプローチによる内視鏡アシスト下での第1肋骨切除および斜角筋切離術を施行している. 内視鏡を併用する利点は, 直視下法では確保しにくい手術野がモニターへ映し出されるため神経血管束を視認しながら確実に第1肋骨および斜角筋切離が可能な点である. さらに合併症に関して, 直視下法では気胸・胸膜損傷の合併症は約15%に発生するが, 本法では3%と気胸・胸膜損傷の合併症発生率を下げることができ, 安全な手術方法と考える. 我々はこれまで肩関節周囲痛を主訴に来院された患者がTOSの術後に肩関節周囲痛や可動域が劇的に改善する症例を経験している. そのため, 肩関節周囲痛に対しては, 肩関節内病変のみならずTOSも鑑別に入れて診察することが重要と考える. 当院は他院で診断に難渋する症例も多く来院されるが, 最後の砦と考え手術を施行している. 本稿では, 症例を提示し当院での診断から手術, さらに後療法までの流れを紹介する. |
