| Abstract | 「Abstract」肩甲上神経は肩甲切痕, 棘窩切痕など骨, 靱帯組織に囲まれた狭い部位を走行するため, オーバーヘッドスポーツなどでの肩の運動による圧迫や牽引, またガングリオン, 広範囲腱板断裂などの外的要因の影響を受けやすい. 棘上筋, 棘下筋の筋力低下とともに, 肩の後方から外側, 腋窩にかけて感覚障害が認められることが多いが, 筋力低下を示す筋肉と感覚障害範囲は麻痺の発生部位によって異なるため, 神経の走行部位のどの位置で障害されているかの推測に役立つ. また術前CTは肩甲切痕の形状や靱帯骨化の有無の確認に, MRIはガングリオンの位置の確認などに有用である. 手術治療としては最近は鏡視下手術が主流となっており, 侵襲が少なくガングリオンや関節唇損傷など, 関節内の評価や処置が同時に可能であるという利点がある. 肩甲切痕での肩甲上神経剥離術では, 肩峰前内側角と肩甲棘内側端を結んだ中点 (SNポータル) より肩甲切痕の位置を確認し, 肩甲上動脈をメルクマールに上肩甲横靱帯を同定する. |
