| Abstract | 「Abstract」足部の絞扼性神経障害には内果後方で発症する足根管症候群, 足背に発症する前足根管症候群および前足部に発症するMorton病が挙げられる. 神経が絞扼されている部位において疼痛を伴い, それぞれの神経が支配する領域で知覚障害を認める. 足根管症候群では距踵骨癒合症やガングリオンが原因となることが多く, 外傷や軟部腫瘍により引き起こされることもある. 治療には足根管を開放し, 原因となる要因を取り除くことで症状の緩和が得られる. 前足根管症候群はガングリオンによる神経の絞扼が原因となることが多く, 前足根管開放術が適応される. Morton病は足部の底側趾神経が慢性的に圧迫されることにより生じることが多いが, 神経腫により発症することもある. 診察には圧迫による疼痛誘発試験であるThumb index finger squeeze testやMulder testを用いる. 治療にはステロイド剤の注射や神経切断術, 神経剥離術などが用いられる. |
