| Abstract | 「Abstract」骨盤輪骨折・寛骨臼骨折の治療では早期の良好な機能回復を得るためには外傷蘇生を終えた時点で適切な術前評価のもと治療を進める必要がある. 骨盤輪骨折に対しては経皮的スクリュー固定法, プレート固定法, spinal instrumentを用いた固定方法があるが骨折型に応じて使い分ける必要があり, 術者はそれぞれの手術方法に精通しておく必要がある. 寛骨臼骨折は関節面の整復が外傷後関節症のリスクを軽減させるとされ, 手術の際には解剖学的整復位を目指しアプローチを含めた綿密な術前計画を立てたうえで準備を行い, 手術に臨まなくてはならず難度も高い手術である. 寛骨臼骨折の治療成績には手術症例の経験数も影響があるとも考えられるため, 必要であれば経験ある術者の手術応援・見学などを考慮すべきである. 術後も後療法を進めつつ異所性骨化に注意しその予防に努め, 長期にわたり慎重に経過を見ていく必要がある. |
