| Abstract | 「Abstract」三角線維軟骨複合体 (TFCC) 損傷は老若男女, 活動性を問わず手関節尺側部痛の原因疾患となりうる. その機能は手関節尺側, 遠位橈尺関節の安定支持機構ならびに衝撃緩衝体である. TFCC損傷の診断はulnocarpal stress testなどの各種理学所見MRIなどの画像所見によりなされる. 損傷型は従来Palmer分類が用いられてきたが, 鏡視下手術の発達に伴い様々な損傷型が明らかとなっており, その多様性を認識する必要がある. 保存治療に反応しない疼痛は手術の対象となり, 手術の多くは鏡視下手術で行われる. その方法は血行動態, 関節安定性の観点から, 原則としてTFCC実質部は掻爬, 周辺部損傷は縫合・再建の適応となる. また画像, 鏡視所見にて尺骨突き上げの所見がある場合は尺骨短縮術の併用を考慮する. |
