| Abstract | 「Abstract」肩甲骨骨折手術体位は健側下側臥位で行う. 肩峰骨折には肩峰-肩甲棘進入で容易に進入できる. 関節窩-体部骨折への進入は, Judet approachとBrodsky approachに大別され, 各種変法が考案されている. 肩甲骨には専用ロッキングプレートが存在しないため, 筆者は肩峰骨折にはVA-LCP-DHP posterior-lateral plateを, 体部骨折にはMatrix Mandible plate 2.4mmを使用している. 低侵襲なBrodsky approachから広範展開可能なJudet approachへの変更は皮膚血流の観点から通常はできない. しかし, Brodsky approachで手術を開始して骨折整復困難であった症例において, Reverse incision extensile Judet approachに変更することにより, 広範囲展開で解剖学的整復固定が得られ成績も良好であった. |
