| Abstract | 「Abstract」 関節リウマチにおいて高度に破壊され疼痛と変形をきたしたPIP関節病変に対して, より良い手指機能の獲得を目指して人工関節置換術が考慮されることがある. 隣接関節の状態, 罹患指(橈側指か尺側指か), 随伴するスワンネック変形・ボタンホール変形の程度などにより適応を厳格にし, 感染症や変形再発などの合併症に注意して持続的な術後フォローを行う必要がある. 人工関節の機種はシリコンインプラントが主流であると思われるが, それ以外の機種も使用されている. 筆者が主に使用してきたSelf Locking Finger jointの手術手技と治療成績を概説した. |
