| Abstract | 「Abstract」術後の化膿性肩関節炎の発症頻度は低いが, 治療に遅れ関節破壊をきたすと著しい機能障害を残す. 発症早期の診断と早期の外科的治療の介入が重要であり, 関節鏡視下洗浄・デブリドマンにより低侵襲に病巣部の十分な洗浄と郭清を行うことができる. 感染を鎮静化させるためには, 適切な抗菌薬投与を行いながら術後の効果的なドレナージの併用が有用である. 術後の関節液の再貯留を避けるために開放性ドレナージを併用しているが, 感染が長期化した難治例では, 骨軟部感染症領域において近年注目されている持続局所抗菌薬灌流(continuous local antibiotics perfusion:CLAP)の併用が有効な可能性がある. 術後の化膿性肩関節炎は, 発症早期に適切な診断と治療を行い, 良好な関節機能を温存することを治療目標として, 個々の症例に応じた適切な感染症治療を行うことが重要である. |
