| Abstract | 「Abstract」仙腸関節障害の特徴を知ると, 見つけ出すのは容易であり, この診断ができると腰痛診療はかなり楽になる. 標準的な仙腸関節障害の診断法は, one fingerテストで上後腸骨棘を指す腰殿部痛があり, うつ伏せで仙腸関節部を圧迫する(sacroiliac joint shearテスト)と疼痛が誘発され, 仙腸関節後方靱帯ブロックで70%以上疼痛が軽快することである. 一連の外来診療のなかでは, エコーを用いた仙腸関節後方靱帯ブロックが診断と治療に有用である. 仙腸関節ブロックを1〜2週間ごとに複数回行って疼痛強度を下げつつ, 仙腸関節機能の改善を目指す. 関節の不適合の改善には徒手療法が有効で, 骨盤ベルトの装着で関節を安定化させる. 続いて腹部体幹筋トレーニングを行い, 日常生活での負荷に耐え得る仙腸関節の剛性を得る. 難治例では, 仙腸関節腔内の炎症, 仙腸関節周囲靱帯付着部症にまで進んでいることがある. また, 難治化の要因には腰椎疾患・股関節疾患との合併もある. 仙腸関節ブロックの効果が持続しない場合には, 腰椎や股関節の検索が必要で, それらへの治療をあわせて行うことで仙腸関節への治療効果が促進される例もある. |
