Abstract | [Abstract] 大腿骨近位部骨折は超高齢社会となった日本において年々増加しており, 整形外科の重要なテーマである. 受傷に伴い, 生命予後と機能予後の低下が起こることが知られている. その予後と関連する因子を, 大腿骨近位部骨折のタイムコースに沿ってまとめた. 患者背景因子として, 高齢である・認知症がある・併存症が多い・受傷前のADLが低いことが予後不良因子となる. 骨折型でいえば, 転子部骨折のほうが頚部骨折よりも機能予後が悪い. 周術期の管理に関しては, 麻酔による予後の差異は大きくなく, 老年内科と協力して患者を術前から管理することで生命予後・機能予後をともに改善できる可能性が高い. 手術までの待機期間はクリアカットなボーダーラインはみられないが, 各国のガイドラインでなるべく早期に行うことが推奨されている. |
