| Abstract | [Abstract] 手根管を完全に開放したにも関わらず, 症状が改善せず治療に難渋する症例をrecalcitrant carpal tunnel syndromeと呼ぶ. こうした症例に対する再手術方法として, 近年は血流の良い軟部組織(flap)で神経を被覆する治療が試みられるようになり, 優れた臨床成績が報告されるようになった. 我々は神経の被覆範囲が手首皮線を越えず, 滑膜の量が豊富な場合は滑膜弁を, 乏しい場合は小指球脂肪弁を選択している. また, 神経の被覆範囲が手首皮線を越えて近位に広がる場合は, 尺側もしくは橈側の前腕筋膜脂肪弁を選択している. 次に, 重度の手根管症候群に対する機能再建だが, 母指球筋萎縮が著明で術前からつまみ障害を強く訴える手根管症候群に対し, 我々は積極的に母指対立再建術を併用している. この際, 橈側の横手根靱帯にプーリーを作製することで移行腱(長掌筋腱)の走行方向を変えたCamitz変法を行っている. 本法は移行腱が正中神経を横切らず, なおかつ移行腱の走行が対立再建にとって理想的な方向に近づくため, 比較的安心して利用できる効果的な手法であると考えている. |
