| Abstract | 「はじめに」ステロイドは, ステロイド骨格という構造をもつ化合物の総称である. 生体内で産生されるステロイドには副腎皮質で産生される性ホルモン, 糖質コルチコイド, 鉱質コルチコイドなどがある. また, ステロイドホルモンの基質となり, 細胞膜を構成する一要素でもあるコレステロールも, ステロイド骨格をもつ生体内ステロイドの一種である. 臨床の場ではステロイドホルモンを配合した薬品や, 生理的な副腎皮質ホルモン(ヒドロコルチゾンやコルチゾンなど)にさまざまな修飾を加えることでその性質や活性能, 薬理動態などを変化させた, 合成ステロイドが広く使用されている. われわれ麻酔科医も術中のステロイドカバーや術後悪心嘔吐(postoperative nausea and vomiting: PONV)の予防, アナフィラキシーや喘息発作の治療など, 周術期をはじめとして, ペインクリニックや緩和医療領域など, 使用する機会が少なくない. |
