| Abstract | 「はじめに」常染色体顕性(優性)多発性嚢胞腎(autosomal dominant polycystic kidney disease:ADPKD)では, 遺伝子変異により両側腎臓に多数の嚢胞が進行性に発生・増大し, 腎臓以外の種々の臓器にも障害が生じる. 嚢胞は多臓器にわたり, 肝臓, 膵臓, 精巣, くも膜に好発する. 肝嚢胞の頻度は女性で高く, 経産婦では肝嚢胞が増大傾向となるといわれている. ADPKD の85%以上に肝嚢胞がみられる. 常染色体潜性(劣性)多発性嚢胞腎(autosomal recessive polycystic kidney disease:ARPKD)はADPKDより稀であり, 肝嚢胞の合併の頻度は低い. 一方で常染色体顕性多嚢胞性肝疾患(autosomal dominant polycystic liver disease:ADPLD)の発症は10万人に1人程度と見積もられており, ADPKD合併の多発性肝嚢胞に比べると頻度ははるかに低い. |
