| Abstract | 「はじめに」糖尿病性腎症は糖尿病の細小血管合併症の1つで, 末期腎不全へ進展する慢性腎臓病(chronic kidney disease: CKD)の主要な原疾患である. 2023年10月, 糖尿病に関連する慢性腎臓病の呼称に関して「diabetic kidney disease」に対応する日本語訳が「糖尿病関連腎臓病」とされ, 新たな腎臓病の概念が定義された. 糖尿病, 糖尿病に関連する病態, 併存疾患, あるいは治療の影響などを含め, 糖尿病症例が一生のなかで経験し, 糖尿病状態が病態に影響を及ぼしている可能性がある慢性腎臓病が包含されている. なかでも, 糖尿病症例特有に発症する糖尿病性腎症は, 糖尿病関連腎臓病のなかで最も重要である. glucagon-like-peptide-1(GLP-1)受容体作動薬は, わが国では2010年6月にリラグルチドが発売となり, 以降エキセナチド, リキシセナチド, デュラグルチド, セマグルチドが次々と上市され, 糖尿病合併症診療では欠かせない薬剤となった. |
