| Abstract | 「はじめに」尿路感染症は, 尿道から細菌が尿路に侵入する逆行性感染により引き起こされる非特異的炎症である. 臨床的には基礎疾患の有無に基づき, 単純性と複雑性に分類される. 単純性尿路感染症は通常, 20〜40代の性的活動期の女性に多くみられ, 主な原因菌は大腸菌である. 一方, 複雑性尿路感染症は尿路の通過障害や全身性の基礎疾患に続発してみられ, 小児では尿路奇形に起因することが多い. 腎盂腎炎は, 細菌が腎実質へ侵入し炎症が惹起されて発症し, 症状を呈する. 問診時に膀胱炎症状を事前に自覚していることも稀ではない. 抗菌薬で改善が不十分な際には, 遷延し, 時に重症化する症例も経験する. 本稿では, 上部尿路感染症である急性腎盂腎炎, 急性巣状細菌性腎炎, 腎膿瘍について解説する. 「I 急性腎盂腎炎」「1. 概要と分類」腎盂腎炎は, 細菌感染により腎盂および腎杯に炎症が生じる疾患で, 主に上行性尿路感染により惹起される有熱性尿路感染症である. |
