| Abstract | 「はじめに」血管径の違いから, 血管炎は大血管, 中血管, 小・微小血管炎に分けられる. 抗好中球細胞質抗体(anti-neutrophil cytoplasmic antibody:ANCA)関連血管炎は全身の中, 細・小血管の免疫複合体の沈着がみられない壊死性血管炎を病態とし, ANCAが発症に深く関与している. ANCAには現在, PR3(proteinase 3)-ANCA, MPO(myeloperoxidase)-ANCAの2種類の抗体価が主として測定されている. 耳鼻咽喉科領域と関連の強いANCA関連血管炎は, 多発血管炎性肉芽腫症(granulomatosis with polyangiitis:GPA, 旧名:Wegener肉芽腫), 顕微鏡的多発血管炎(microscopic polyangiitis:MPA), 好酸球性多発血管炎肉芽腫症(eosinophilic granulomatosis with polyangiitis:EGPA, 旧名:Churg-Strauss症候群, アレルギー性肉芽腫性血管炎)の3つに分けられる. 本稿では, GPA, EGPAの耳鼻咽喉科領域と関連した症状, 診断, 治療について述べる. |
