| Abstract | 「はじめに」鼻副鼻腔疾患は炎症性, 腫瘍性ともに詳細に分類すると多岐にわたる. 解剖学的に頭蓋内・眼窩内合併症を含めた重篤な病態にいたることがあり, 注意を要する領域である. 診断, 合併症の精査, 病変の伸展範囲の把握において, 画像診断の意義は大きい. 本稿では, 鼻副鼻腔における代表的な疾患の画像所見を解説する. 「急性鼻副鼻腔炎」ウイルスまたは細菌性に, 鼻副鼻腔内の粘膜肥厚や膿汁貯留をきたす疾患である. 発症4週間以内の鼻副鼻腔炎を急性鼻副鼻腔炎と称する. 症状は膿性鼻漏, 後鼻漏, 鼻閉, 前頭部痛, 頬部痛などを呈する. 一側性のことが多く, CTでは副鼻腔内の軟部陰影を認めるが, 特異的な所見ではない. 液面を示唆する所見や内部の泡沫を認めることがある. 合併症として, 頭蓋内, 眼窩内への感染波及がある. 頭蓋内への波及により, 硬膜外膿瘍, 脳膿瘍, 髄膜炎, 脳炎, 海綿静脈洞血栓症などをきたすことがある. 眼窩内の炎症にはChandler分類があり, Group 3以上であれば緊急手術を考慮する. |
