| Abstract | 「はじめに」先天性水腎症と膀胱尿管逆流(vesicoureteral reflux: VUR)は, 小児泌尿器科のなかでもっともよく遭遇する疾患である. しかしその管理方法については, いまだコンセンサスが得られていない問題も多くある. それぞれの管理方法について概説する. 「I. 先天性水腎症」水腎症とは腎盂・腎杯が尿の停滞や逆流などにより拡張した病態である. 先天性水腎症の原因としては腎盂尿管移行部通過障害(ureteropelvic junction obstruction: UPJO)によるものがもっとも多く, ついでVUR, 膀胱尿管移行部通過障害, 後部尿道弁などによるものがある. 原因は種々あるが, 一般に小児先天性水腎症といえば頻度のもっとも多いUPJOを意味する. UPJOの病因として内因性と外因性や, 両者を明確に区別できず, 重複しているものもある. |
