| Abstract | 「はじめに」直腸肛門奇形には鎖肛や総排泄腔遺残などの疾患が含まれ, その頻度は1,500〜5,000出生に1人と報告されている. 鎖肛は出生後早期に治療が必要となる頻度が高いため, 出生前診断が望ましい. しかし, 鎖肛の出生前診断は難しいとされている. 以前は胎児の画像検査による腸の拡張所見を診断の糸口にしていたが, 近年, 超音波診断装置を用いて肛門括約筋の観察をすることで鎖肛の出生前診断率が上がるという報告がみられるようになってきた. また, 鎖肛はほかの構造異常と合併することが多く, そうした疾患と合わせて出生前診断され, 出生後の治療計画を立てることが望まれる. 「I. 鎖肛の出生前診断」近年の鎖肛の胎児診断は, 超音波診断装置を用いた肛門括約筋の「ターゲットサイン」を描出する方法を用いた報告が多い. |
