| Abstract | 「はじめに」出生前診断された血管腫・血管奇形・リンパ管奇形のなかで, 周産期に救命のために特別な治療を要する症例には, (1)巨大な肝血管腫, (2)頭頸部・縦隔のリンパ管腫〔(一般型・嚢胞性)リンパ管奇形(common(or cystic)lymphatic malformation)またはlymphangioma〕がある. 新生児の巨大な肝血管腫は腫瘤による圧排性の呼吸障害, 高拍出性心不全, 凝固障害, DICを起こし, 死亡率も比較的高い. 頭頸部・縦隔の気道に接するリンパ管腫では, 出生後に気道圧迫による呼吸障害をきたす可能性がある. これらの巨大な病変は出生前診断の進歩により周産期のリスクを正確に診断し, 状況に応じた分娩法の選択や, 出生後の診療チームの早期の形成ができるようになってきている. しかし一方で血管腫・血管奇形・リンパ管奇形のなかには, 胎児期・出生直後の正確な疾患の鑑別が困難で, 治療方針決定の際に注意を要するものもある. |
