| Abstract | 「はじめに」短腸症候群は, 短小腸に加え, 通常の栄養管理では水分, 電解質を含む各栄養素の吸収が不十分で必要量が満たされない, 腸管吸収不良症候群といえる. 短小腸のさまざまな原因に加え, 残存小腸の長さ以外に腸管の切除部位や腸管の吻合部位, 人工肛門の有無, 回盲弁の有無などにより, 栄養素の吸収不良には差がある. また, 臨床経過や腸管順応の状況によってもその吸収能には差が生じる. さらに小児では発達・発育への配慮も必要で, 年齢に応じた栄養投与量の調整も重要となる. 近年, 短腸症候群を含む腸管不全患者に対して, 欧米では多職種支援の腸管リハビリテーションが普及している. 短腸症候群の腸管リハビリテーションは, 患者のQOL改善を目指した栄養管理が主体となる. 栄養障害を有する短腸症候群患者に対しては, 適切な静脈栄養を基とした栄養療法, 薬物療法, 外科的療法, 社会支援など包括した栄養管理を行う必要がある. |
