| Abstract | 「はじめに」移動性精巣と停留精巣の鑑別に迷った経験のない小児科・小児外科・泌尿器科医はいないだろう. ポピュラーな疾患にもかかわらず, これほど診断・治療方針が曖昧にならざるをえない疾患は珍しい. 本稿では疾患ガイドラインに書かれていない診療のポイントをできるだけ具体的に述べることで, 読者の日常臨床の一助となることを目指したい. なお, あくまでもナラティブな視点・現場目線で執筆しており, 現時点で判明しているエビデンスについてはわが国の停留精巣診療ガイドラインの他, 多くの既報告を参照していただきたい. 「I. 移動性精巣・挙上精巣の定義」わが国の「停留精巣診療ガイドライン」では, 「移動性精巣 (移動精巣・遊走精巣) とは陰嚢内の上端から底部までを, あるいは陰嚢内から鼠径部までを容易に移動する精巣の状態であり, 多くは精巣挙筋の自然な収縮によって移動すると考えられている」とある. |
