| Abstract | 「はじめに」 腸重積症は時間的経過や病状の程度により生命の危険や重篤な合併症を引き起こす疾患である. そのため, 診察前の状況や, 病院での非観血的整復の成功率は, 治療の結果に大きく影響する. 国外の文献の検討・評価には, それぞれの国の社会や医療の状況を考慮する必要がある. 本稿では, 2022年に発行された『エビデンスに基づいた小児腸重積症の診療ガイドライン 改訂第2版』(以下, ガイドライン)と最新の文献からの情報を報告する. 「I. 疫学」 ガイドラインによると, ロタウイルスワクチン接種により, 腸重積症の発生リスクは軽度上昇する可能性がある. わが国における2020年の市販後調査によると, Rotarix(R)において腸重積症発生率は10,000人あたり4.74件/年, Rotateq(R)においては100,000人あたり159.08件/年であり, 自然発生の範囲内と判定された. ロタウイルス感染症自体が腸重積症の発生リスクを上昇させることから, ワクチンにより感染と腸重積症の発生抑制効果が期待できること, ロタウイルス腸炎を抑制する効果が腸重積症発生のリスクを上回っていることから定期接種化に至っている. |
