| Abstract | 「はじめに」 炎症性腸疾患(inflammatory bowel disease : IBD)は, 下痢, 腹痛, 血便などを主訴とし, 消化管に慢性炎症がみられる疾患である. 主に潰瘍性大腸炎(ulcerative colitis : UC)とCrohn病(Crohn's disease : CD)に分類されるが, 小児期発症のIBDは, UCやCDの診断基準を満たせないことで確定診断に至らず, 分類不能型IBD(IBD unclassified : IBD-U)と診断されることがある. さらに近年, 6歳未満で発症する超早期発症型IBD(very early onset IBD : VEO-IBD)のなかには, IBD特有の消化器症状のみならず, 単一遺伝子疾患として免疫不全症などの基礎疾患を有する場合があることが明らかになってきており, 診断と治療において特に注意が必要である. 「I. 診断」 UCとCDにおけるわが国における診断基準に関しては, 難治性疾患政策研究事業「難治性炎症性腸管障害に関する調査研究」(久松班)が報告している「潰瘍性大腸炎・クローン病診断基準・治療指針」に記載されており, 小児IBDを疑う症例を診断する際には必ず参照すべきである. |
