| Abstract | 「はじめに」 日本では従来米国式の治療法, すなわちまず腫瘍摘出を行い, 手術所見・病理所見に従い術後化学療法・放射線治療を行ってきた. 最近JCCG(日本小児がん研究グループ)の腎腫瘍委員会では, SIOP-RTSG(The Renal Tumour Study Group of the International Society of Paediatric Oncology)との国際共同研究を開始した. SIOPの治療方針はまず化学療法を行い, 腫瘍の縮小をはかったあとに腫瘍の摘出を行うが, その詳細はSIOP-UMBRELLAプロトコールに「標準治療ガイドライン」として記載されているので参照されたい. 本稿では, SIOP式の治療を行う場合に小児外科医が留意すべきポイントを解説する. 「I. 限局性Wilms腫瘍に対する術後の治療方針」 限局性Wilms腫瘍は, 術後病期リスク・病理診断および化学療法後の腫瘍体積に従って, 術後の化学療法・放射線治療を行う. |
