| Abstract | 「要旨」プロトンポンプ阻害薬(PPI)長期投与と関連する胃ポリープとして, 胃底腺ポリープ, 過形成性ポリープ, 多発白色扁平隆起(腺窩上皮ポリープ)がある. 胃底腺ポリープはおもに H. pylori 陰性の胃底腺領域にみられる. 特に水腫様に増大した胃底腺ポリープはPPI長期投与例に特異的であり, PPIの直接作用や高ガストリン血症で増大すると推測されている. 過形成性ポリープはおもに H. pylori 現感染または既感染の胃内全部位にみられ, 多発白色扁平隆起は H. pylori 感染の有無と関連なく胃底腺領域にみられる. 過形成性ポリープと多発白色扁平隆起は, PPI長期投与例に特異的ではなく, PPIによる高ガストリン血症が作用を及ぼし増大すると推測されている. 胃底腺ポリープ, 過形成性ポリープが顕著に増大した場合はまずPPIの減量・休薬を検討する. 径が大きく表面の凹凸不正を伴う胃底腺ポリープは, 内視鏡的切除を考慮しつつ経過観察を行う. 過形成性ポリープは出血や癌化のリスクがあるため, H. pylori 除菌療法や内視鏡的切除を行う必要がある. |
