| Abstract | 「要旨」Collagenous colitis (CC)は, 慢性下痢をきたし病理組織学的に大腸の上皮基底膜直下に膠原線維束がみられる疾患であり, 通常内視鏡下の生検で診断される. 病因はなお明らかではないが, 欧米における複数の症例対照研究では薬剤がCCの発症リスクを高めることが示され, 特にプロトンポンプ阻害薬(PPI)の関与が注目されている. 本邦では欧米と比べると頻度が低いが, 薬剤(特にPPI)が関連した症例が大半を占める. CCはもともと, 画像上異常がみられないことが特徴とされていたが, 以下のさまざまな内視鏡所見が報告されている. (1)色調変化: 発赤, 発赤斑, 褪色など, (2)血管像の変化: 血管透見低下・消失, 血管増生など, (3)表面性状の変化: 浮腫, 易出血性, 粗ぞう粘膜, 顆粒状粘膜, 偽膜, 粘膜裂創, ひび割れ様所見など, (4)その他: ハウストラ消失など. 特に粘膜裂創と顆粒状粘膜は本症を疑ううえで重要な所見である. |
