Abstract | 「はじめに」 胆道閉塞に対する内視鏡治療は, 種々のデバイスや胆管ステントが使用可能になってきたことや, さらに超音波内視鏡(endoscopic ultrasound : EUS)を用いたEUS下胆道ドレナージ術(EUS-guided biliary drainage : EUS-BD)の登場により非常に多彩化してきているが, 現在においても胆道ドレナージの基本は, やはり内視鏡的逆行性膵胆管造影法(endoscopic retrograde cholangiopancreatography : ERCP)である. しかし, 術後再建腸管や悪性十二指腸閉塞症例など, 乳頭アプローチ困難な症例においてERCPはchallengingであり, 代替療法が必要となる場合がある. EUS-BDはこのような症例に対して恩恵がある一方で, 胆汁性腹膜炎や, ステントの腹腔内迷入などに代表される偶発症の頻度も高く報告されている. |
