| Abstract | 「はじめに」Helicobacter pylori感染の減少, 食生活の欧米化, 人口の高齢化などさまざまな要因により胃食道逆流症 (gastroesophageal reflux disease : GERD) は増加しており, 日々の消化器内視鏡診療において高頻度に遭遇する病態となっている. GERDの診断は症状と内視鏡所見の組み合わせからなされるものであり, 内視鏡診断と症状とが必ずしも相関しないことが問題として取り上げられている. また, 近年では従来のプロトンポンプ阻害薬 (proton pomp inhibitor : PPI) より強力で即効性のあるカリウムイオン競合型アシッドブロッカーであるボノプラザンフマル酸塩がGERDの日常診療に広く用いられている. 症状が速やかに改善する可能性があるものの, 内視鏡を行うタイミングによっては症状と内視鏡所見がより乖離する症例を経験する. |
