| Abstract | 「はじめに」 新生児GBS(group B Streptococcus)感染症は, 母体に対するGBSスクリーニング検査, GBS保菌者に対する分娩時抗菌薬投与により減少したが, 今もなお, GBSは新生児から乳児における侵襲性感染症の原因菌の主要な微生物である. 本稿では, その疫学, 分類, 臨床像, 治療法について述べる. 「GBSとは」 GBSは, Streptococcus agalactiaeというグラム陽性の溶血性レンサ球菌で, Lancefieldの分類でB群に属する. 血液寒天培地に3〜4mmのコロニーを作り, 比較的小さいβ溶血を示す. 「疫学」 GBS感染症は届け出疾患ではないため日本の正確な疫学は不明であるが, 国立感染症研究所病原微生物検出情報(IASR)では, 生後3か月未満の罹患率(/1000出生)は, 早発型0.08, 遅発型0.10と報告されている. 全国約500か所の基幹病院定点から毎週報告される5類感染症「細菌性髄膜炎(髄膜炎菌, 肺炎球菌, インフルエンザ菌を除く)」のなかでは, GBSは主要な起因菌の一つである. |
