| Abstract | 「はじめに」 炎症性腸疾患(inflammatory bowel disease: IBD)の診断にあたり, 潰瘍性大腸炎(ulcerative colitis: UC)とCrohn病(Crohn's disease: CD)の診断基準(本特集別稿参照)に記載のある画像検査は, UCであれば消化管内視鏡検査と注腸X線検査, CDであれば消化管内視鏡検査である. したがって, 今日の「IBD診断」における画像検査の中心は, 消化管内視鏡検査であることは曲げることのできない事実であり, 今後, 注腸X線検査は小腸内視鏡検査などの発達に伴い, 件数が減少していくことが予想される. しかしながら, 小児におけるIBD診断とその後の「フォローアップに関する画像検査」は考えを異にする必要があり, IBDのフォローアップには, 1977年に国際放射線防護委員会が示した基本指針であるALARA (as low as reasonably achievable) conceptにあるように必要最小限の放射線被ばくにより画像評価を行うよう心がける必要があることや, 消化管内視鏡検査を行う際に, 体動や疼痛を抑制するための鎮静あるいは全身麻酔を行うことに対するリスクが生じることなどを鑑みる必要があることから, 消化管内視鏡検査以外の画像検査の特徴を十分に理解し, それらを適材適所に利用する必要がある. |
