| Abstract | 「はじめに」 5-アミノサリチル酸(5-aminosalicylic acid: 5-ASA)は, 炎症性腸疾患(inflammatory bowel disease: IBD), とくに潰瘍性大腸炎(ulcerative colitis: UC)において, 寛解導入療法および寛解維持療法の基本となる薬剤である. クローン病(Crohn's disease: CD)でも, 効果は限定的ではあるが, 使用される. 2016年に, リアルダ(R)(2020年5月時点では小児には保険適用なし)が加わり, わが国で使用できる5-ASA製剤は4種類となった. また錠剤・顆粒製剤・坐剤・注腸剤といった幅広い剤形があるため, 処方する際には, それぞれの製剤の特徴を理解する必要がある. 本稿においては, 各種5-ASA製剤の概説と, 小児治療指針での位置づけ, 副作用(とくに不耐・アレルギー)について概説する. 「作用機序」 5-ASAは, 結核の治療に使われた抗菌薬であるパラアミノサリチル酸(4-ASA)の位置異性体である. |
