| Abstract | 「はじめに」 潰瘍性大腸炎, Crohn病などの炎症性腸疾患(inflammatory bowel disease: IBD)の患者数は近年増加傾向にあるため, 新規治療薬の研究も盛んに行われている. そのなかでステロイドはキードラッグの一つとして重要な位置にある. 小児におけるステロイド療法は中等症〜重症例の寛解導入療法に用いられる. また, 小児では病期進行が速くて重症例が多いという特徴もあるためステロイドの全身療法が必要となる場合が多い. 成長期である小児にステロイドを使用する際は, 効果や副作用を正確に把握していく必要がある. 「作用機序」 ステロイドの作用は, 抗炎症作用や免疫抑制作用が主体とされる. プロスタグランジンやロイコトリエンを抑制することで血管透過性を減少させて炎症部位への白血球の遊走と貪食を抑制する. また, 活性化Tリンパ球などのリンパ球の産生を抑制する. さらに, 単球, マクロファージに対して抗原提示能を抑制して炎症を鎮静化させると考えられている. |
