| Abstract | 「Key Points」(1) 熱源不明の乳児に対して抗菌薬投与が必要と判断した場合には, 投与前にカテーテルによる採尿を行い検尿と尿培養を実施してUTIの鑑別を行う. (2) 膿尿が認められなくてもUTIは除外できない. また膿尿を認めてもUTIでないことがある. (3) VURを合併する複雑性UTIにおいては, E. coli 以外の腸内細菌, Enterococcus 属, P. aeruginosa の割合が増え, また耐性菌の比率も高まる. (4) 初回のUTIを疑った場合は E. coli を標的として, 第1〜第3世代のセフェム系抗菌薬を投与する. (5) 上部UTIを起こした小児においては, VURなどのCAKUTやBBDの有無を評価する. |
