| Abstract | 「はじめに」新生児ループス(neonatal lupus erythematosus:NLE)は, おもに母体が保有する抗SS-A/Ro抗体, 抗SS-B/La抗体などの自己抗体が関与し, 胎児や新生児, 乳児に心病変, 皮膚病変, 血球減少, 肝機能異常などを引き起こす後天性の自己免疫症候群である. 胎児の心伝導系と皮膚組織はSS-A抗原を発現しており, 胎盤を通過した母体由来の抗SS-A抗体の結合により臓器障害をきたすと考えられている. なお, 新生児ループスやNLEという用語は国内外で広く使用されているが, 患児が全身性エリテマトーデス(systemic lupus erythematosus:SLE)に罹患しているわけではなく, 母親も自己抗体を保有しているのみで症状がなく健康な場合も多いうえ, SLE以外の自己免疫性疾患を発症している場合もあるため, 誤解のないよう注意が必要である. |
