| Abstract | 「1 基本病因, 発症機序」わが国の小児の片頭痛有病率は3.5〜5%とされているが, 世界各国の小児片頭痛の有病率は4.0〜19.2%とさまざまである. 男女比は小児期には性差が乏しく, 思春期以降にホルモンの影響で女性が明らかに多くなる傾向にあり, 生活支障度も高くなる. 周期性嘔吐症候群 (cyclic vomiting syndrome : CVS) の有病率は0.3〜4%程度とされ, 10歳ごろから周期性嘔吐症候群から片頭痛に移行し, 約8割が18歳までに片頭痛に移行するといわれている. 明らかな性差はないが, 小児期ではやや男子に多い傾向にある. 家族性片麻痺性片頭痛など特殊は片頭痛に関しては, CACNA1Aなどの遺伝子が同定され診断に有用であるが, 一般的な片頭痛やCVSの特定的な遺伝子は同定されていない. |
