| Abstract | 「1 基本病因, 発症機序」自己免疫性溶血性貧血 (autoimmune hemolytic anemia : AIHA) は, なんらかの理由によって出現した赤血球に対する自己抗体により, 赤血球が破壊されて起こる後天性溶血性貧血であり, 表1のように分類できる. 小児期では, 低年齢児に感染後の冷式抗体によるAIHA, 思春期以降で温式抗体によるAIHAが多い. AIHAの80%が温式抗体によるものとされる. 日本小児血液・がん学会の疾患登録では, 年間10〜20例の登録がある. 「2 基本病態」AIHAの各病型で認められる自己抗体の特性, 臨床像の特徴と治療について表2にまとめた. 温式AIHAで認められるIgG型抗体は, 37℃が至適反応温度であり, 補体結合能は弱いため, 血管内溶血はまれである. |
