| Abstract | 「I. 細菌性髄膜炎」「[1] 疾患概念」「1)定義」細菌性髄膜炎は, 医療の発展した現代においても死亡5%, 後遺症15%を認めるもっとも重篤な細菌感染症の一つである. 2013年にインフルエンザ菌b型(Haemophilus influenzae type b: Hib)ワクチン, 肺炎球菌結合型ワクチン(pneumococcal conjugate vaccine: PCV)が定期接種化されて以降, 肺炎球菌性髄膜炎は70%, インフルエンザ菌性髄膜炎は98%罹患率が低下したことが報告されており, 現在では10万人に1人と白血病よりも少ない罹患頻度となっている. しかし, その重篤さや迅速な治療開始が必要であることから疾患の重要性は変化していない. 「2)発症機序」菌血症から細菌が血液脳関門を突破することによって細菌が髄腔内に浸潤する場合, 隣接臓器の炎症(副鼻腔炎, 中耳炎, 乳様突起炎など)から細菌が浸潤する場合, 手術(主に脳室腹腔シャント術)や外傷・髄膜瘤などによって外部と髄腔が交通することによって細菌が直接浸潤する場合がある. |
