| Abstract | 「[1] 疾患概念」「1)微生物学的な分類」血液寒天培地に生えたコロニー周囲に強いβ溶血を起こすレンサ球菌を指して溶血性レンサ球菌(溶連菌)と呼び, 病原性が高いことが多い. Lancefieldの分類では, A群, B群, C群, G群などがある. A群溶連菌(group A streptococci: GAS)であるStreptococcus pyogenes, B群溶連菌(group B streptococci: GBS)であるStreptococcus agalactiaeが小児科で遭遇する代表的な病原菌である. Streptococcus anginosus group(SAG)の中には, 多様な溶血パターンを呈するレンサ球菌があって, 一部, β溶血性である. 「2)GAS」GASは, 細菌による直接的な感染症, 細菌が産生する毒素性疾患, 免疫応答による二次性疾患をひき起こす. 小児では軽症疾患の頻度が高く, 急性咽頭炎, 急性扁桃腺炎などの気道感染症, 伝染性膿痂疹, 蜂窩織炎などの皮膚軟部組織感染などがある. |
