| Abstract | 「[1] 疾患概念」わが国の細菌性食中毒の原因で薬物治療の対象となるのは, カンピロバクター, サルモネラ(非チフス性)が多い. 本稿ではこれら2菌種に加え, 頻度は少ないが臨床的に重要なエルシニア, 腸管出血性大腸菌と赤痢, 院内発症の腸炎としてクロストリディオイデス・ディフィシルを中心に述べる. 細菌性腸炎のなかで, 腸管毒素が原因となる黄色ブドウ球菌やウェルシュ菌については抗菌薬治療の対象とはならず本稿では扱わない. 胃腸炎症状を呈する小児で細菌性腸炎を示唆する症状として, 突然発症の1日4回以上の下痢, 下痢発症前の嘔吐がほとんどない, 血便, 高熱などがある. 一方で, サルモネラ(非チフス性)やエルシニア(Yersinia enterocolitica)では消化器症状を伴わない菌血症の病型, エルシニア(Yersinia pseudotuberculosis)では虫垂炎や川崎病に類似した病型を呈することがある. |
