| Abstract | 「[1] 疾患概念」新生児発作(neonatal seizures)は, 新生児の中枢神経障害を示唆する重要な徴候で, 速やかで正確な診断・病因特定・治療介入が必要となる. 新生児発作を生じさせた原疾患の脳への影響に加え, 新生児発作自体が児へ悪影響を及ぼし, さらなる脳障害を助長する可能性もある. 新生児発作の診断には脳波検査が必須であり, そのうえで(1)臨床症状を認めるもの(electro-clinical)(with clinical sign)と, (2)臨床症状を認めないもの(electrographic-only)(without clinical signs)とに大きく分けられる. 「[2] 診断への道筋」新生児発作は臨床症状と発作時脳波所見の乖離(electro-clinical dissociation)が特徴であり, 臨床症状のみでの判断は困難である. 新生児発作の診断の鍵は脳波検査の施行である. NICUでは脳機能モニタリングとしてaEEG(amplitude integrated EEG)が普及している. |
