| Abstract | 「[1] 疾患概念」免疫性血小板減少症(immune thrombocytopenia: ITP)は, B細胞から産生される抗血小板抗体による血小板の「破壊亢進」と「産生低下」によって血小板減少を生じる自己免疫性疾患で, 小児の血小板減少症のなかでもっとも頻度が高い. 基礎疾患のない一次性ITPと基礎疾患に続発する二次性ITPに分類される. 本稿では「一次性ITP」を「ITP」として論じる. 抗血小板抗体が産生される機序の一つは, 小児では発症前に先行感染を伴うことが多く, ウィルス感染などを契機として, 血小板表面上の抗原とウィルス抗原に交差反応する抗体が産生されることが挙げられる. 一方で, 自己の血小板抗原を非自己として認識する機序も想定されている. 本来, 抗原提示細胞に貪食された血小板抗原由来のほとんどのペプチドは自己の主要組織適合性複合体(major histocompatibility complex: MHC)クラスII分子に結合しない潜在性ペプチドとなる. |
