| Abstract | 「[1] 疾患概念」血球貪食性リンパ組織球症(hemophagocytic lymphohistiocytosis: HLH)は, 以前は血球貪食症候群(hemophagocytic syndrome: HPS)とよばれていた症候群である. HLHは, 発熱や肝脾腫などの臨床症状と, 汎血球減少, 高トリグリセリド血症, 低フィブリノーゲン血症, NK細胞活性低下, 高フェリチン血症, 可溶性インターロイキン2受容体高値などの異常検査所見によって特徴づけられ, 骨髄やリンパ節では血球貪食像を伴う組織球の浸潤が認められる. HLHは発症の原因によって, 単一遺伝子異常による「一次性HLH」と, 他疾患に続発する「二次性HLH」に分類される. 「一次性HLH」として, 細動障害性顆粒放出にかかわるパーフォリン遺伝子の異常をはじめとした家族性HLH(Familial HLH)が含まれている. |
