| Abstract | 「I. 甲状腺機能低下症」「[1] 疾患概念」甲状腺機能低下症は, 血中の甲状腺ホルモンが低下し, 生体の各組織の代謝が低下した状態である. このうち, 胎生期または周産期に生じた甲状腺の形態または機能異常による先天的な機能低下症が先天性甲状腺機能低下症(congenital hypothyroidism: CH)であり, 橋本病を代表とする後天的な機能低下を後天性甲状腺機能低下症と称する. また甲状腺に異常がある原発性甲状腺機能低下症と視床下部・下垂体に異常がある中枢性甲状腺機能低下症に分類される. 甲状腺機能低下症の代表的な疾患と病因を表に示す. 甲状腺ホルモンは胎児期, 乳児早期の神経発達に必須のホルモンであり, このホルモンの作用不足により神経細胞の障害を引き起こし, 重症例では精神運動発達の遅れをきたす. そのほか成長障害, 遷延性黄疸, 便秘, 活気低下などさまざま症状を呈する. |
